My husband bakes the challah in our house. He has made a ladder shape on several occasions. All he does is make two long ropes for sides and a number of thinner shorter ropes for rungs. Put the rungs down on the baking sheet first, then brush the ends of each rung with egg wash and lay the side ropes down over these. Then egg wash the whole thing.
There may be more sophisticated methods to shape ladders, but this works, and is very recognizable as a ladder.
Be careful when removing it from the baking sheet and storing, as the challah may end up falling apart if you're not.
